• Constituted 13 January 1941 in the Regular Army as the 45th Medical Battalion (Armored) and assigned to the 3d Armored Division
• Activated 15 April 1941 at Camp Beauregard, Louisiana
• Redesignated 1 January 1942 as the 45th Armored Medical Battalion
• Inactivated 10 November 1945 in Germany
• Activated 30 July 1948 at Fort Knox, Kentucky
• Redesignated 1 October 1957 as the 45th Medical Battalion
• Inactivated 15 December 1984 in Germany
• Redesignated 45th Support Battalion (Forward) and activated 1 May 1987 in Germany using personnel and equipment from the 2d Support Battalion(Forward)
• Inactivated 15 December 1991 at Hanau, Germany
